Athletic Physicals Policy
ALL STUDENTS participating in a sport must update the following paper work annually: The parent/guardian consent form, medical history form, and physical evaluation form. The parent / guardian, medical history, and physical evaluation are to be completed before any practice (both in-season and out-of-season) or games / matches. The forms of "Parent/Guardian consent" (HM260.1a) "Medical History"(HM620.1b,c), and "Physical Evaluation" (HM620.1d) are to be used by all schools. Updated for 2010-2011! Athletic physicals are required on all students participating in sports. All students participating in sports currently are required to have sports physicals.

Where to get sports physicals:

• Student's own physician or clinic
• Urgent care clinics do sports physicals for about $40.00.
